My friend has been diagnosed with stage 4 colorectal cancer which has spread to his liver. . Advice?My best friend has been diagnosed with cancer.

Here is his diagnosis. He is 55 and except for this has almost never been sick. He was the vision of health before his diagnosis.

?Patient has been diagnosed with a malignant neuroendocrine tumor arising in the
rectum metastatic to liver and lymph nodes, stage IV?

two doctors have said he has 6-8 months to live.

His oncologist said last week basically chemo is not worth it, it might ad two months to his life but ruin the time he has left, and to prepare for going into a hospice to die soon.

Here are my concerns:

First, his oncologist was with an HMO, Kaiser Permanente. He lives near one of the best cancer hospitals in the country, University of California San Francisco. They have trial programs that might include him.

Would an HMO be more likely to ?write him off??

Would be benefit from getting a second opinion at a research hospital?

Is it worth fighting? Or is his diagnosis really that hopeless? I see survival stories on the web.

His wife is Chinese and is taking him to see Oriental doctors. Can they help?

Any advice would be appreciated. I fly out to California tomorrow to be with him as he gets a second opinion.
I want to thank you all so much for your help. I will be going to the oncologist with my friend tomorrow.

People beat cancer sometimes. Stage 4 cancer, not so much. It sounds like any type of treatment could be helpful. If the doctors are right, it won?t matter if treatments don?t work.

There isn?t likely to be much difference in the second opinion.

Metastatic cancer in the liver is generally terminal. 6 ? 8 months may be being optimistic.

Chemo is only used in cases like this to delay the inevitable. Generally it can only slow the progression and not stop or reverse it. Given the side effects from the chemo, most patients would opt to feel relatively well for whatever time they have left rather than feeling ill for a few months longer.

Sorry not to be able to offer more hope, but I have seen to many people die once the cancer takes hold in the liver.

your concern is very understandable and your friend is lucky to have you however be careful that you don?t push this too much with him?2 years ago my best friend was diagnosed with breast cancer and 7 months to live. i was devastated and fought it until i realised that my stress for her was making it hard for her to enjoy the time she had. respect your friends wishes and be his best friend in this time were he needs it most

You?re a good friend to be so concerned. Typically, metastatic cancer (spreading) to the liver and lymph nodes is not a happy diagnosis. It seems your friend is moving forward with alternative medication and seeing a new doctor. Personally, I wouldn?t go through all of the chemo and radiation because it does affect your quality of life. These treatments leave the patient weak, listless, achey and nauseated. But, each person approaches these challenges differently. I don?t think an HMO would ?write him off? as doctors who specialize in this area are really ?special?. They understand and their staffs are specially trained to deal with sick and grieving patients and families. Perhaps you are asking for how YOU should approach him knowing the circumstances? In that case, just listen and love him and his wife and do what you can. That?s probably all he really wants is to know someone will care for him as he goes through all of this strife and remember him once he?s gone. I wish everyone facing something like this could have a friend like you. Godloveyouall.

Do some research on Avastin aka Bevacizumab and colorectal cancer. The drug has been approved for Colorectal cancer and has provid some good results and the side effects are tolerable. Also look for drug trials on http://www.clinicaltrials.gov/ct2/home. I would write off this oncologist and try to get a different oncologist.

I am stage IV melanoma and was on Avastin for two years along with several chemos. They are questioning Avastin?s use for melanoma and breast cancer but works great for colorectal cancer.

I assume these Chinese doctors provide alternative solutions for cancer pain ect. His wife is doing all she can to help him after what the oncologist stated. There is still radiation and surgey options.

Your friend has a fairly rare cancer as 98% of cancers above the anal verge are adenocarcinomas. Stage 4 colon cancer is not curable regardless of histology, but neuroendocrine tumors are harder to treat and have a poorer prognosis. What your friend was told is true it is not due to the HMO. The median survival for a stage 4 neuroendocrine colon cancer is 5-7 months. A clinical trial may provide some benefit.

I just got over stage 3, and I read everything I could about the stages. At stage 4, the average survival rate I?ve seen is a 5% chance of living more than 5 years (cancer.gov is a reliable source for information). I don?t think the HMO would just ?write him off,? even though I don?t trust them either. A second opinion is always a good idea, and I would listen to the oncologist more than anyone on the web. If he tolerates the chemo well, he might be willing to try it. I didn?t have many problems at all?I never got sick, never lost my hair, no mouth sores, anything, but he might consider how it affects him vs. how much more time it will buy him (i.e., quality vs. quantity).

Try asking about ?Avastin?.
Its a new drug and quite effective against advanced colorectal cancer. It works different to chemotherapy and has few side effects! Instead it uses the immune system to attack the tumor blood supply starving and shrinking tumors.

It is very sad to see your friend suffering or dying,it happened that my aunt died from this disease (she had leukemia).I do not know your friend?s personality,such a diseases is hard to resist even to a strong person.If he is a believer or is an adept of nontraditional medicine,it will be easier for him to survive ;moreover try to give him a hope because the hope is the last one.
